School Based Health Center

If you are interested in scheduling an appointment for your child, please complete the School Based Health Center registration form on the Child & Family Agency website. You can also call 860-437-4550 for assistance.

For immunizations, please complete the registration form and consent to vaccinate. Be sure to select which vaccines your child is due to receive.
For physicals, please complete the registration form, Bright Futures according to age, and pediatric symptom checklist (PSC).
For sick visits, registration form only.
